Grace Healthcare Services is seeking an experienced and compassionate Advanced Practice Nurse (APN) to provide high-quality palliative care services throughout Newark, NJ, in partnership with a major health system. This program integrates hospital-based, skilled nursing facility (SNF), home-based, and telehealth palliative care services to ensure seamless patient-centered care across settings.

This position is ideal for a clinically skilled palliative care provider who thrives in interdisciplinary collaboration, symptom management, and delivering patient-centered care across diverse settings within Ocean County.

Advanced Practice Nurse - Palliative Care Program

Full Time or Part Time

Location: Newark, New Jersey

Qualifications:

● Current Licensure as an Advanced Practice Nurse in the State of New Jersey.

● 3 years or willingness to train as a Nurse Practitioner in clinical nursing care of Palliative Care, Oncology, Adult/Family Practice, or Geriatrics preferred.

● CDS, DEA, BLS & ACLS Certifications or comparable experience in palliative care

● Medicare billing privileges as a nurse practitioner.

● Strong clinical skills in symptom management and end-of-life care.

● Excellent communication and interpersonal skills.

● Ability to work collaboratively in a multidisciplinary team.

● Compassionate and empathetic approach to patient care.

Job Description:

● Conduct comprehensive assessments of patients' medical, psychological, and social needs.

● Develop and implement individualized care plans in collaboration with the palliative care team.

● Manage symptoms such as pain, nausea, and shortness of breath to improve patients' quality of life.

● Prescribe medications and treatments as appropriate.

● Provide emotional and psychosocial support to patients and their families.

● Facilitate communication amongst healthcare team members and patients/families

● Advocate for patients' wishes and ensure their care aligns with their goals and values.

● Coordinate care with other healthcare providers to ensure continuity and comprehensive care.

● Support the continuity of palliative care plans by assisting with discharge planning

● Perform accurate and timely billing of APN services
